Before You Attend Your Appointment

All appointments must be booked online. Each person must have their own individual booking.

To help us prepare for your visit, all patients are required to complete a Travel Risk Assessment Form as part of the booking process.

If you are travelling to a high-risk area where Yellow Fever certification is required, you must also complete the Yellow Fever Risk Assessment Form before your appointment.

Before Your Appointment

 
  • Bring any previous vaccination records with you. You can get these from your GP or the NHS App.

  • Not sure what you need? Book an initial consultation. The nurse can give all required vaccines on the same day, subject to availability.

  • If you’re booking for more than one person, book each person separately, then attend together at the earliest time.

  • If you need more than one vaccine, book for one and list the others in the comments section.

 

Specific Requirements

 
  • Yellow Fever certificate reissue: Bring proof of previous vaccination.

  • Cholera vaccine: Do not eat or drink for two hours before and two hours after the vaccine.

  • Malaria tablets: We do not supply these. You can buy them over the counter at a pharmacy.
